Insurance
Session rates for self-pay clients are $179 for initial sessions and $149 for follow-up sessions. Insurance rates may vary from private pay rates depending on the insurance provider and your insurance plan.
We do offer a limited number of spots of sliding scale slots, please schedule a discovery call to discuss availability and eligibility further.
We accept credit cards, debit cards, HSA, and FSA cards for all balances not covered by insurance or if you are a self-pay client.
We are in-network with Blue Cross Blue Shield, Anthem, Aetna, and Medcost. At this time we are unable to accept out of state HMO plans (this includes Blue Home and Blue Local) or Medicare/Medicaid plans run through commercial insurers (Medicare Advantage, Healthy Blue, etc.). Our practice will run a benefit check prior to your first scheduled appointment or discovery call, but there are are situations where we are unable to check benefits for specific plans (example: Blue Cross Blue Shield of SC). In this case we will request that you check your benefits using the questions below.
If you are covered by another insurance company we am more than happy to provide you with a superbill to submit to your insurance company for reimbursement for our sessions. The superbill does not guarantee reimbursement.
To check your insurance benefits, call the members service number on the back of your insurance card and ask:
2. Does my plan cover outpatient nutrition counseling? (CPT codes: 97802 and 97803)?
- If yes, how many session are allowed?
- Does my plan cover nutrition counseling as a preventative service? (Diagnosis Code: Z71.3)
- Does my plan only cover nutrition counseling to treat medical conditions I have like ___________? (examples: type 1 or type 2 diabetes, heart disease, eating disorder, etc.)
- If located outside of NC - does my plan cover telehealth for providers located out of state?
- If yes, how much is my deductible?
- How much of the deductible have I met?
6. Do I need a physician referral?
Note: Make sure to record the representative's name and reference # when checking your benefits. This information will be necessary if you ever need to dispute a rejected claim.
